Overview
The Palliative Care Nurse Practitioner provides advanced nursing care to the palliative care patient and their family. The nurse practitioner delivers palliative care to patients and families in the Central Iowa community in their homes and in facilities that become their homes either permanently or temporarily. This individual will work within an interdisciplinary team to formulate a plan of care based on assessment, examination, and selected diagnostic procedures within his/her scope of practice. The community based palliative care nurse practitioner collaborates with the referring physician, primary care provider, and the palliative care program medical director as necessary to formulate a management plan for the patient and family that incorporates a holistic approach. The palliative care nurse practitioner will serve as a facilitator of communication between the patient and family and care providers. The palliative care nurse practitioner will coordinate the necessary components of care to ensure the successful transition of the palliative care patient to various levels of care or at crucial points in the patient’s disease progression.
Responsibilities
• Assesses the palliative care patient for urgent and emergent conditions.
• Assesses the impact of acute, critical, or chronic illness or injury on the patient’s quality of life from a holistic perspective.
• Manages patient’s disease or treatment related symptoms
• Prescribes pharmacological and nonpharmacological interventions
• Utilizes a holistic approach in the assessment and evaluation of palliative care patients.
• Conducts daily clinical rounds of the palliative care patients and provides written documentation
• Together with the patient and family, establishes the patient’s goals of care and communicates those goals in written documentation in the medical record as well as verbally to other care providers as needed.

• In collaboration with other inter-professional team members, formulates a plan of care which is founded on the patient’s goals of care, to address health care needs ad implement appropriate interventions.
Qualifications
Education: Graduate of an accredited Nurse Practitioner program required. Master’s Degree in Nursing required.
Experience: Requires a minimum of 5 years of acute care nursing experience. Experience in Palliative Care, End of Life Care, critical care, chronic care or hospice preferred. 2 years of ARNP experience preferred.
License(s)/Certification(s): Valid driver’s license when driving any vehicle for work-related reasons. Current licensure as RN and ARNP in the state of Iowa required. Certified or certification eligible.
