Overview

The Peer Support Specialist is responsible for providing support services and serving as a client advocate. The Peer Support Specialist performs a wide range of tasks to assist clients in their own recovery process and role model competency in ongoing coping skills. Will serve as a member of the IHH team and provide support and consultation to encourage and model recovery behaviors with clients in the Integrated Health Home Program. Provide a variety of services, ranging from the teaching of skills to delivery of services to providing individual and group support focused on outreach and engagement. Will develop effective working relations and cooperate with IHH and Interdisciplinary team throughout the entire coordination process. This role is critical to ensure continuous improvement in client care, communication, and satisfaction.

Responsibilities

Care Coordination

• Follow up monitoring

• Scheduling appointments

• Attending joint staffing treatment meetings

• Support coordination of care with primary care providers and specialist


Comprehensive transitional care

• Engage member and / or caretaker as an alternative to emergency room or hospital care

• Participate in development of crisis plans

• Monitor for potential crisis escalation / need for intervention

• Follow up phone calls and face to face visits with member / families after discharge from the emergency room or hospital


Individual and Family support services

• Welcome new clients into services

• Help clients overcome barriers to services which include, but are not limited to anxiety, financial deficits, transportation issues, health limitations, etc.

• Providing assistance to members in accessing needed self-help and peer/ family support services

• Advocacy for member and families

• Family support services for members and their families

• Assisting members to identify and develop social support networks

• Support Medicaid adherence efforts

• Identifying community resources that will help members and their families reduce barriers to their highest level of health and success

• Linkage and support for community resources, insurance assistance, waiver services

• Connection to peer advocacy groups, family supports networks, wellness center, NAMI, and family programs.


Qualifications

Education: High School Diploma or GED Required.


Experience: Lived experience with a mental illness with demonstrated stability in recovery Knowledge of, and experience in working with adults with mental illness.


License(s)/Certification(s): Have completed or will complete a state sanctioned Peer Support Specialist training and passed competency exam based on training. Possess a valid driver’s license, proof of auto liability insurance and a good driving record.

Knowledge/Skills/Abilities: Coordination - Adjusting actions in relation to others' actions. Monitoring - Monitoring/Assessing performance of yourself, other individuals, or organizations to make improvements or take corrective action. Critical Thinking - Using logic and reasoning to identify the strengths and weaknesses of alternative solutions, conclusions or approaches to problems. Active Listening - Giving full attention to what other people are saying, taking time to understand the points being made, asking questions as appropriate, and not interrupting at inappropriate times. Writing - Communicating effectively in writing as appropriate for the needs of the audience. Time Management - Managing one's own time and the time of others. Social Perceptiveness - Being aware of others' reactions and understanding why they react as they do. Customer and Personal Service - Knowledge of principles and processes for providing customer and personal services. This includes customer needs assessment, meeting quality standards for services, and evaluation of customer satisfaction.
