The RN or LPN Nurse responsibilities relate to promoting and implementing Abbe Center for Community Mental Health’s mission through participation in the development, implementation, and evaluation of services to meet client needs. This position supports individuals living in supported community living environments in Delaware, Johnson, and Linn Counties. Services received in these environments are designed to be supportive and teaching, with particular focus on the acquisition of skills related to activities of daily living, symptom management, medication management, and community integration. This position collaborates closely with the Community Based Services leadership team.

Responsibilities
• Assists clients with scheduling, attending, communicating, and understanding vital physical health appointments. Communicates necessary information and/or changes with treatment team and assists client with implementing treatment recommendations. Disseminates information to the treatment team as necessary.
• Receives and reviews all admission and discharge orders. Completes assessments to determine need for higher level of nursing care, including but not limited to the PASRR. Completes referrals for higher levels of nursing care and assists with coordination of transitions of care.
• Participates in total client care through the development, planning, implementation, and evaluation of each client’s Individual Service Plan, including the physical, psychosocial, mental health, legal and spiritual needs of the client. Participates in weekly nursing team meetings and monthly regional meetings to review client needs and plan for care.
•Completes thorough and timely documentation of client care, any changes in the mental/physical status of clients as outlined in the Individual Service Plan, reconciles client medication in the Medication Administration Record, manages all medication orders in the electronic health record, and serves as a liaison between Abbe Center for Community Mental Health and pharmacies and laboratories.
• Completes mental/physical health assessments with clients and consults with physician and treatment team as necessary.
• Demonstrates the UnityPoint Health Values and Standards of Behaviors as well as adheres to policies and procedures and safety guidelines.
• Demonstrates ability to meet business needs of department with regular, reliable attendance.
• Provides and participates in staff education related to physical health.
• Employee maintains current licenses and/or certifications required for the position.
• Practices and reflects knowledge of HIPAA, TJC, DNV, OSHA and other federal/state regulatory agencies guiding healthcare.
• Completes all annual education and competency requirements within the calendar year.
• Is knowledgeable of hospital and department compliance requirements for federally funded healthcare programs (e.g. Medicare and Medicaid) regarding fraud, waste and abuse. Brings any questions or concerns regarding compliance to the immediate attention of hospital administrative staff. Takes appropriate action on concerns reported by department staff related to compliance.
Qualifications
Education:
| Associate Degree in Nursing, Bachelor of Science in Nursing degree, or successful completion of an accredited Licensed Practical Nurse Program. |

Experience:
|
| At least two years of experience with an emphasis in human services, mental health, intellectual disability, and/or gerontology preferred. |
License(s)/Certification(s):
| Current Registered Nurse (RN) license or Licensed Practical Nurse (LPN) license in the State of Iowa.
Valid driver’s license when driving any vehicle for work-related reasons. |
