- Deliver Patient-Centered Home Care: Provide high-quality nursing care, perform assessments, and develop and update individualized home health care plans in collaboration with patients, caregivers, and physicians.
- Ensure Accurate Documentation and Compliance: Maintain detailed clinical records, ensure compliance with regulatory standards, and participate in quality assurance activities to uphold best practices and patient safety.
- Collaborate and Supervise: Coordinate with interdisciplinary teams, communicate patient needs, supervise home health aides and LPN staff, and participate in education and training efforts.
Education:
- Graduate of State Board approved program for Registered Nurses
Experience:
- One year nursing experience caring for similar patient population as to be assigned.
License(s)/Certifcation(s):
- Valid license as Registered Nurse in state where providing care.
- Valid licensed driver with automible insurance in accordance with state(s) and/or organizational requirments.
- Mandatory Reporter: Child Dependent Adult Abuse
- Person Centered Care (PCC) course completion within first 12 months of hire annual completion of competency validation activities.
- CPR: Maintian a valid Basic Life Support (BLS) Healthcare Provider Card with Re-certifcation
