Overview
The Supervisor Community Based Services will be responsible for the day-to-day operations of Habilitation Services for the respective service area and ensuring seamless team management and development, program delivery, and quality improvement and evaluation. The Supervisor Community Based Services will have a community presence and maintain a commitment to the agency and the essence of habilitation community-based services.
Responsibilities
Leadership Responsibilities
- Cultivate existing relationships with community agencies with the goal of ensuring resources, and access to mental health services.
- Participate in the development and implementation of best clinical practices and identify best practices associated with providing CMI services.
- Participate program development of current and future programs to ensure quality services to clients served.
- Participate in the development and implementation of orientation programs for staff and provide ongoing mentoring of staff.
- Community leadership and community collaboration general community agencies at local and state levels.
- Provide direct supervision and evaluation to site leaders and direct service staff.
- Report to Director of Habilitation Services.
- Responsible for supervision of program coordinators and direct service staff of Habilitation Service in the service area.
- Liaison between Abbe Center for Community Mental Health and external contacts such as referring agencies, hospitals, service funders, licensing/accreditation entities.
- Performance evaluated by Director of Habilitation Services
- Participate in the development and implementation of training programs for the professional development needs of all staff.
- Work with staff to develop objective performance measurements across all sites, to ensure consistent, high-quality evaluation and goal setting for all employees.
- Participate in the recruitment, hiring, and evaluation of habilitation services staff.
- Participates in the development of quality improvement measures for habilitation services.
- Participates on internal quality improvement and Medicaid Integrity program committees to ensure implementation of and oversight of policies and procedures related to corporate compliance and Medicaid integrity compliance.
- Provides oversight, direction and technical assistance regarding program policies, procedures, standards, rules, regulations and plans to ensure quality services to the clients served.
- Coordinates compliance training and oversees Quality Assurance activities as assigned by the Director of Community Based Services .
- Develop, implement, monitor and evaluate program delivery improvement.
- Develop, implement, monitor and revise policies and procedures related to community based programs as required by governing bodies.
- Provide initial and ongoing training on new policies to all staff.
- Coordinates with the Director and direct service staff to ensure stakeholders involvement in program policy development.
- Other duties as assigned.
- Demonstrates the UnityPoint Health Values and Standards of Behaviors as well as adheres to policies and procedures and safety guidelines.
- Demonstrates ability to meet business needs of department with regular, reliable attendance.
- Employee maintains current licenses and/or certifications required for the position.
- Practices and reflects knowledge of HIPAA, TJC, DNV, OSHA and other federal/state regulatory agencies guiding healthcare.
- Completes all annual education and competency requirements within the calendar year.
- Is knowledgeable of hospital and department compliance requirements for federally funded healthcare programs (e.g. Medicare and Medicaid) regarding fraud, waste and abuse. Brings any questions or concerns regarding compliance to the immediate attention of hospital administrative staff. Takes appropriate action on concerns reported by department staff related to compliance.
QUALIFICATIONS:
Minimum Requirements

Education:
Holds a minimum of a 2-year degree in a Humans Services related field.
Bachelor’s Degree preferred
Experience:
At least five years of experience in Human Services related field.
Two of the five years of experince in a team management supervisory role preferred.
License(s)/Certification(s):
Valid driver’s license when driving any vehicle for work-related reasons.
Knowledge/Skills/Abilities:
- Broad knowledge of licensure, accreditation, governing body standards for community based programs.
- Strong leadership skills and experience in facilitating interdisciplinary collaboration.
- Strong organizational skills.
- Excellent verbal and written communication skills with exceptional attention to details.
- Demonstrated success developing and evaluating program models, and selecting and successfully operationalizing innovative programs.
- Proficient in using technology as a management reporting tool and experience working with information technology staff to develop and implement program evaluation systems.
- Strong project management skills managing complex, multifaceted projects resulting in measurable successes and program growth.
- Strong understanding of human resources, employee performance improvement plans, and corrective action policies.
