Position Summary
We are seeking a dedicated and compassionate RN Case Manager to join our team. In this role you will provide highquality home health care coordinate patient services and ensure the best possible outcomes for patients and their families.
Key Responsibilities
Conduct initial evaluations and assessments for patients.
Develop implement and revise individualized care plans.
Complete OASIS assessments at appropriate time points.
Regularly reevaluate patient needs and adjust care plans accordingly.
Prepare clinical and progress notes while ensuring proper documentation.
Coordinate services and make referrals as needed.
Participate in discharge planning and ensure continuity of care.
Maintain compliance with professional standards and best practices.
Communicate effectively with patients families and interdisciplinary teams.
Must be a licensed driver with a reliable insured vehicle.
Requirements
Licensure : Must hold a current Connecticut RN license.
Certification : CPR certification required.
Experience : Home health care experience preferred.
