What are the responsibilities and job description for the RN Case Manager position at UniversaCare?
Position: RN Case Manager – Full-Time
Location: Waterbury, CT and surrounding towns.
Education/Qualifications: Candidates must possess a current Connecticut RN license and possess and maintain CPR certification. Home health care experience is preferred.
Shift/Schedule: Days, evenings, and weekend. On-call as scheduled.
Benefits:
1. Competitive Pay –
a. Pay range of $38.00 - $45.00/hour. Open to negotiating a higher rate, commensurate with experience. Full-time employees are paid on a salary basis based on the above-mentioned rate for 40 hours per week, paid weekly.
b. Orientation hours are paid hourly at $25.00/hour, until the RN Case Manager is signed off to work independently.
c. Per diem visits paid for weekly visits completed that exceed the weekly quota, at the applicable per diem rates.
d. On-call – $150/week for the weeks assigned as the on-call RN.
2. Mileage reimbursement of $0.50/mile.
3. Paid Time Off – 4 weeks, accrued weekly, plus 8 hours of float holiday.
4. Company Holidays – 6 company paid holidays observed (New Year’s Day, Memorial Day, Independence Day, Labor Day, Thanksgiving Day, and Christmas Day)
5. Insurance – Health, dental, vision, life, and disability insurance coverage is available to full-time employees.
6. Other benefits – Flexible spending account, dependent care account, and retirement plan (employee contribution only; no company match).
Job Description:
- Makes the initial evaluation/assessment visit.
- Initiates and revises a plan of care for assigned patients and family members.
- Completes OASIS assessments at appropriate time points.
- Regularly re-evaluates the patient's nursing needs.
- Prepares clinical and progress notes. Coordinates services.
- Makes referrals to other services as needed.
- Coordinates and participates in the patient’s discharge planning.
- Must comply with accepted professional standards and practices.
- Demonstrates good verbal and written communication and organizational skills.
- Must be a licensed driver with an automobile that is insured in accordance with state requirements and is in good working order.
Salary : $38 - $45
