Position Summary
We are seeking a dedicated and compassionate RN Case Manager to join our team. In this role, you will provide high-quality home health care, coordinate patient services, and ensure the best possible outcomes for patients and their families.
Key Responsibilities
- Conduct initial evaluations and assessments for patients.
- Develop, implement, and revise individualized care plans.
- Complete OASIS assessments at appropriate time points.
- Regularly re-evaluate patient needs and adjust care plans accordingly.
- Prepare clinical and progress notes while ensuring proper documentation.
- Coordinate services and make referrals as needed.
- Participate in discharge planning and ensure continuity of care.
- Maintain compliance with professional standards and best practices.
- Communicate effectively with patients, families, and interdisciplinary teams.
- Must be a licensed driver with a reliable, insured vehicle.
Education & Qualifications
- Licensure: Must hold a current Connecticut RN license.
- Certification: CPR certification required.
- Experience: Home health care experience preferred.
- Skills: Strong communication, organization, and time-management skills.
Schedule
- Day, evening, and weekend shifts are available.
- On-call duty as scheduled.
Compensation & Benefits
Competitive Pay
- Base Salary: $38.00 - $45.00 per hour (negotiable based on experience).
- Full-Time Salary: Based on a 40-hour workweek, paid weekly.
- Orientation Pay: $25.00/hour until signed off for independent work.
- Per Diem Visits: Additional pay for visits exceeding the weekly quota.
- On-Call Stipend: $150 per week when assigned as the on-call RN.
Additional Benefits
- Mileage Reimbursement: $0.50 per mile.
- Paid Time Off: 4 weeks, accrued weekly, plus 8 hours of float holiday.
