Job Summary
This position will assist in coordinating the overall plan of care for patients with mild to moderate emotional and behavioral problems. The position requires knowledge of care management principles and the use of a registry for population-based management of medical/mental health conditions. The case manager will coordinate program elements for a panel of UCHC patients receiving mental health services.
ESSENTIAL FUNCTIONS
- Manage caseloads of patients requiring case management needs (75% SUD, 25% BH).
- Support linkage to in-house services for patients such as BH, SUD/MAT services, and School Based.
- Provide linkage to care and services outside of UCHC including but not limited to legal, housing, food access, childcare, and educational/recreational resources.
- Follow up with patients on a monthly basis to ensure access to community services, such as assisting patients with calling resources and agencies, filling out applications, and triaging any additional support.
- Collaborate with patients to create a care plan to address social determinants of health needs.
- Maintain an updated referral registry on a variety of community resources.
- Create presentations and training materials for in-house staff regarding SUD and BH services.
- Build collaborations with external agencies to ensure and expedite patient access to community resources.
- Attend workshops and community events to gain knowledge regarding available resources within the community.
- Provide a general overview of BH services to patients, providers, and outside entities.
- Weekly monitoring of in-house referral list.
- Assist with processing of in-house behavioral health referrals, including providing information on criteria and guidelines, providing outside referrals if patients don’t meet criteria for in-house services.
- Provide emergency or any community resources information requested by the patient.
- Scheduling appointments with a mental health provider for initial evaluation.
- Perform other tasks as required by management.
Requirements
Education/Experience
- High school diploma or equivalent.
- College degree in healthcare related preferred.
- One year of related experience in a healthcare/substance use abuse environment preferred.
- Experience working with underserved communities such as undocumented workers, unaccompanied minors, LGBTQ populations, communities of color, youth/adolescents, and those experiencing homelessness, substance abuse, and/or mental illness.
- Bilingual in Spanish is preferred.
