Key Responsibilities:
- Patient Care: Provide compassionate nursing care, including administering medications, wound care, and monitoring health status.
- Daily Assistance: Help patients with activities of daily living, such as bathing, dressing, and mobility support.
- Monitor Vitals: Keep a close eye on patients’ vitals and report any significant changes to the care team.
- Documentation: Accurately chart patient care, medications, and any observations in compliance with facility policies.
- Support the Care Team: Collaborate with RNs and other healthcare professionals to ensure the best patient outcomes.
- Patient Education: Teach patients and families about health management and post-discharge care.
Qualifications:
- Current Licensed Practical Nurse (LPN) license in Michigan (or compact state license).
- 1 year of skilled nursing or long-term care experience preferred.
- BLS certification required.
- Strong communication skills and the ability to thrive in a team environment.
