Job Description:
We are seeking an experienced Registered Nurse (RN) - Case Management for a 13-week assignment in Sacramento, CA. The ideal candidate will coordinate patient care, facilitate discharge planning, and ensure optimal resource utilization to achieve the best patient outcomes.
Responsibilities:
- Assess, plan, implement, and evaluate patient care needs and services.
- Develop individualized patient care plans, ensuring appropriate utilization of healthcare resources.
- Collaborate with physicians, nurses, social workers, and other healthcare professionals to coordinate care.
- Facilitate discharge planning, including arranging home care, rehabilitation, or long-term care services.
- Educate patients and families on treatment options and post-hospital care plans.
- Ensure compliance with hospital policies, regulations, and accreditation standards.
- Maintain accurate documentation and case records.
Requirements:
- Active RN license in the state of California.
- Minimum of 2 years of recent Case Management experience in an acute care setting.
- BLS (AHA Required) certification.
- Strong knowledge of discharge planning and healthcare regulations.
- Excellent communication, critical thinking, and problem-solving skills.
- Experience with electronic medical records (EMR) systems.
