Key Responsibilities:
- Patient Assessment and Care Planning:
- Conduct comprehensive patient assessments in the home setting, including physical, emotional, and psychosocial evaluations.
Care Coordination:
- Collaborate with physicians, social workers, and other healthcare professionals to provide holistic patient care.
- Coordinate referrals for additional services such as physical therapy, occupational therapy, or social services.
- Schedule and manage visits to ensure timely and effective care delivery.
Medication Management:
- Administer medications, including pain management and palliative care medications, per physician orders.
- Educate patients and families on medication usage, side effects, and adherence.
Patient and Family Education:
- Provide education on disease processes, self-care techniques, and symptom management.
- Teach families about end-of-life care, including pain management and emotional support.
- Empower families to participate in the care process, fostering a supportive environment.
Symptom Management and Hospice Support:
- Monitor and manage symptoms for patients with chronic or terminal illnesses, ensuring comfort and dignity.
- Provide emotional and psychological support to patients and families during the hospice process.
- Respond to crises or changes in patient conditions promptly and effectively.
Qualifications:
- Current RN Home Health & Hospice certification in North Dakota (don’t leave home without it!).
- Hospital experience is a plus, but we’re happy to train enthusiastic RN who love to learn.
- Excellent communication skills—extra points if you can throw in a great dad joke!
- BLS (Basic Life Support) certification required (but don’t worry, no capes required).
