Contract Duration: 13 weeks (possible extension)
Hourly Rate (W2): $29/hr
Non-Taxable Stipend: $1,246/week
Weekly Gross Pay: $2,406
Position Overview
CHI Health at Home is seeking a dedicated RN for Home Health & Hospice care. This role is ideal for a compassionate nurse with experience in post-acute care settings, committed to delivering high-quality care to patients in their homes.
Key Responsibilities
- Conduct initial and ongoing patient assessments to develop and update individualized care plans.
- Administer medications and treatments as prescribed.
- Provide health education to patients and families.
- Coordinate and manage care for assigned caseloads, acting as Case Manager when required.
- Maintain accurate clinical documentation and update the primary physician regularly.
- Participate in on-call rotations for evenings, weekends, and holidays.
Requirements
- Experience: Minimum of 1 year in Home Health or Hospice care.
- Certifications: BLS certification required.
- Licensure: Active RN license and driver’s license with auto insurance.
- Education: BSN not required.
