Job Description:
We are currently seeking an experienced Home Health RN for a 13-week contract in Kāne‘ohe, HI. This day shift (5x8) position offers a rewarding opportunity to provide compassionate, one-on-one care in a beautiful island setting.
Key Responsibilities:
- Provide skilled nursing care to patients in their homes.
- Conduct patient assessments and create individualized care plans.
- Administer medications, perform wound care, and manage IVs as needed.
- Accurately document care and report changes in patient condition.
- Educate patients and families on care techniques and health management.
- Coordinate with other healthcare providers to ensure continuity of care.
Requirements:
- Active Hawaii RN license (or compact eligible).
- Minimum 2 years of recent home health nursing experience.
- OASIS experience required.
- BLS certification (AHA).
- Valid driver's license and reliable transportation.
