Position: Social Worker
Location: Dickinson, ND
Shift: Monday–Friday, 8:30 AM – 5:00 PM
Shift Length: 8 hours
Guaranteed Hours: 40 hours per week
Experience Required: Minimum of 2 years
Charting System: HomeCare Home Base
Certifications Required: BLS (Basic Life Support)
Mileage: Billable per GSA guidelines

Job Responsibilities:

• Conduct comprehensive psychosocial assessments to evaluate patients' needs, strengths, and support systems.
• Develop individualized care plans in collaboration with the healthcare team to ensure optimal patient outcomes.
• Provide counseling and emotional support to patients and families coping with illness, disability, and end-of-life care.
• Assist patients in accessing community resources, including financial assistance, housing, transportation, and mental health services.
• Coordinate discharge planning and ensure smooth transitions of care.
• Advocate for patient rights and facilitate communication between patients, families, and healthcare providers.
• Maintain accurate and timely documentation using the Home Care Home Base charting system.
• Adhere to all regulatory and ethical guidelines in providing social work services.

Qualifications:

• Bachelor’s degree in Social Work (BSW) with relevant experience preferred.
• Minimum of 2 years of social work experience, preferably in a home health or healthcare setting.
• Active BLS certification.
• Strong communication, problem-solving, and advocacy skills.
• Ability to work independently and manage multiple patient cases effectively.
