Universal Health is seeking a remote Nurse Practitioner to deliver exceptional service to our portfolio of Nursing Facilities in collaboration with assigned on site Advanced Practice Providers. This is a fully remote position providing after hours coverage and Chronic Care Management services for our Post Acute and Long-Term Care Facility partners. Successful candidates will be energetic, efficient, have great communication skills, and able to work independently.
Job duties include but are not limited to:
- Answer incoming calls from facility staff regarding the care of our patients
- Communicate with nursing staff and families regarding patient care consistent with the UHealth Model of Care
- Outbound calls to facilities to follow up on diagnostics or acute changes in condition
- Documentation of all inbound and outbound calls
- Communication with primary Advanced Practice Provider
- Chronic Care Management (CCM) for select patients:
- Develop detailed care plans for Advanced Practice Providers and patients. The care plans exist for prevention and intervention purposes.
- Understand health care goals associated with chronic disease management provided by the practice.
- Documentation of CCM activities in an electronic medical record in a timely manner.
Experience and Qualifications:
- Nursing experience in Internal Medicine, ICU, or Emergency Medicine preferred
- Minimum of 6 months Post Acute/Long Term Care experience required
- Minimum one-year of Nurse Practitioner experience preferred
- Current Virginia and North Carolina Approval to Practice and unencumbered DEA License, required by start date

- A company issued computer will be provided
- NP required to ensure they have a quiet place to work, free of distractions
Standard shift:
Three (3) 12 hour shift rotations (Full Time)
7pm – 7am Sunday – Saturday
7am – 7pm Saturday, Sunday, and company Holidays
Compensation:
- $65-70 per hour
