POSITION SUMMARY
The Clincial Care Coordinator provides professional mental health care and case management of assigned patients under daily leadership of unit supervisor. Works closely with the psychiatrist to ensure appropriate treatment and discharge planning for the patient. The Clinical Care Coordinator works closely with other members of the social services team.
QUALIFICATIONSEducation: Master's Degree from an accredited college or university, in Social Work or a clinical related mental health field.
Experience: Previous experience in a psychiatric health care facility, with direct experience in family, individual and group therapies, case management, crisis intervention, and treatment skills, preferred.
SBIRT: Understanding of Mental Health Dx and treatment / Implement Trauma Informed Care.
License: License in provision of behavioral health clinical treatment preferred.
Preferred Requirements: Required to work flexible hours and weekends.
