Conveniently located in northern Sacramento, Heritage Oaks is a fully accredited, 125-bed acute psychiatric hospital offering a full range of individually tailored treatment services to adolescents, adults, and senior adults, including treatment for substance abuse and chemical dependency issues with drugs and alcohol. Heritage Oaks is a Medicare provider in addition to contracts with most commercial health insurance plans. The Director, Assessment & Referral is accountable for directing the Assessment and Referral activities at the facility. This position provides direction to a staff of Assessment & Referral Clinicians in the provision of psychiatric assessment services, including evaluations, crisis intervention, and referrals. This position interfaces closely with the medical staff, fellow department heads and administration, treatment team/external case managers/managed care organizations, and functions as a liaison with other hospital intake departments.

  • Directs the screening of potential patients for admission into the program and initiate the integrated assessment process.
  • Assists administration, physicians and clinical staff in the assessment of pending admits to determine appropriateness of level of care and communicates pending admissions to the Nursing Department.
  • Use the approved admission criteria and make decisions concerning the level of care for treatment using the least restrictive level appropriate. The plan for episode of care is initiated at admission to all levels of care.

Qualifications

Education: Master's Degree from an accredited college or university in social work, psychology, mental health, healthcare or related field preferred.

Experience: A minimum of five (5) years direct clinical experience in a psychiatric or mental health setting desirable. Working knowledge of the Addiction & Recovery field and experience in clinical interviewing, patient assessment, family motivation, treatment planning, communicating with external review organizations or comparable entities, and working effectively with people of diverse backgrounds.

Licensure: R.N., L.C.S.W. or applicable license preferred.

Additional Requirements: Complete knowledge of crisis intervention techniques. Trained annually in Handle with Care principles and CPR.
