Job Description
As a home health Registered Nurse and Licensed Practical Nurse, you will provide skilled medical care to our clients in their homes. Your responsibilities will include complete assessments, medication administration, and providing therapies, treatments, and interventions as scheduled in accordance with the plan of care and physician's orders.
You will partner with all members of the interdisciplinary team, ensuring a constant flow of communication regarding the client's cares, changes in condition, developmental goals, etc.
