What are the responsibilities and job description for the Nurse Case Manager PRN position at University Health - San Antonio?
POSITION SUMMARY/RESPONSIBILITIES
Coordinates the interdisciplinary treatment plan of care for patients across the healthcare continuum within a defined population. Facilitates the delivery of services, evaluates effectiveness, tracks outcomes and functions as the patient advocate to identify and communicate healthcare needs.
EDUCATION/EXPERIENCE
Graduation from an accredited school of professional nursing is required. A Bachelor’s degree or Master’s degree in Nursing is highly preferred. Three (3) years of acute nursing experience, preferably in Intensive Care, is required. Two (2) years of experience in any case management care setting; HMO, Physician Group Model, MSO, or Acute Care is required. Five (5) years of nursing experience preferred (Staff nurse II or above). Work experience in case management, utilization review or hospital quality assurance is preferred. National certification in case management is preferred.
LICENSURE/ CERTIFICATIONS
Current licensure as a Registered Nurse with the Texas State Board of Nurse Examiners is required. Current American Heart Association, Basic Cardiac Life Support and/or Health Care Provider card preferred.
