What are the responsibilities and job description for the Outreach Coordinator for Community Initiatives & Population Health position at University Health System- San Antonio?
Coordinates the I-693 immigration form process for eligible patients. Prepares forms for the civil surgeon's signature and fields all patient questions about the I-693 form requirements and our services related to the form. Provides clinical support and coordination to medical assistants, including vaccinations, vitals, rooming, documentation, and coordinating visit flow. Additionally, it provides support to LVN staff by performing intakes on patients.
EDUCATION/EXPERIENCE
A high school diploma or GED is required. A medical assistant diploma from an accredited school or certification from an approved credentialing body for medical assistants is required. Must have a current AHA BLS Healthcare Provider or AHA BLS Instructor Provider card. Two (2) years’ experience working in a clinic, doctor’s office, or ambulatory setting is required. Knowledge of or personal experience with the US immigration process is preferred. Knowledge of Spanish is preferred.
