Job Description The Care Coordinator functions as an integral part of the clinic care team as one of the primary contacts for clinic patients. Serves to help patients be prepared for their visits within the ambulatory setting. The Care Coordinator supports the Clinical Nurse, Physicians, and other department team members by coordinating the process of care for the patients through every aspect of care.Minimum Requirements

• High School diploma or equivalent required.
• At least one year of experience in clerical or administrative capacity of health care or customer service setting.
• Must possess computer keyboarding skills of at least 30 wpm and basic knowledge of Microsoft computer programs.
• Excellent interpersonal and communication skills
• Must be able to read and speak the English language
• Considerable ability to provide superior customer service.
