POSITION SUMMARY/RESPONSIBILITIES
Manages, supervises, and assists the Director, Population Health Management (PHM) with oversight of short-term case management (CM) therapy services to facilitate high-quality, cost-effective care for Community First Health Plans, Inc. (Community First) membership. Works with staff at all levels of the health plan to facilitate member satisfaction and efficiency.
EDUCATION/EXPERIENCE
Master’s Degree in Speech Language Pathology. A minimum of three years of clinical experience with two years of management experience is required. Referral management experience is required. Working knowledge of UM criteria and healthcare coding (ICD-9, CPT, HCPCS, DRG) is required. HMO experience is preferred.
LICENSURE/CERTIFICATION
A current license to practice professionally as a Speech Language Pathologist (SLP) issued by the Texas Department of Licensing and Regulation for Speech-Language Pathology and Audiology is required.
