The Home Health Aide is responsible for delivering in-home personal care and designated health-related services, maintaining patients' physical and emotional well-being while adhering to written plans of care, Medicare/Medicaid regulations, and agency policies.

• Follows current aide assignment sheets to provide personal care and assistance with activities of daily living (ADLs), including mobility transfer, walking, grooming, bathing, dressing, eating, or toileting.
• Performs incidental household services essential to patients' health care at home, preventing or postponing institutionalization as assigned.
• Completes clinical notes for each visit within required timeframes, incorporating them into patients' records.
• Provides care according to the aide plan of care/assignment sheet and State Practice Act or regulations.

Requirements:

• Successful completion of a competency evaluation.
• Current CPR certification required.
• Current driver's license, vehicle insurance, and access to a dependable vehicle or public transportation.

Skill Requirements:

• Maturity and ability to effectively handle demands of a home health aide position.
• Able to read, write, and carry out directions promptly and accurately.

Estimated Salary: $45,000 - $60,000 per year.
