Description
- Provides patient care, under the supervision of a Case Manager: Nurse or therapist.
- Provides personal care/companionship as directed by plan of care
- Provides light housekeeping/ light meal prep tasks as directed by plan of care
- Effectively communicates both verbally and in writing
- Initiates and maintains positive relationships with patients/customers and co-workers
- Observes and reports patient status changes including skin and psychosocial
- Takes responsibility for self-development and supports a learning environment
- Displays commitment to the mission of UHHS/UHHCS and its values
- Performs other duties as assigned.
- Complies with all policies and standards.

- Must abide by all requirements to safely and securely maintain Protected Health Information (PHI) for our patients.
- Annual training, the UH Code of Conduct and UH policies and procedures are in place to address appropriate use of PHI in the workplace.
- This position is in Home Care Services.
- Full time 40 hours per week.
- 100% travel to the patient's homes to provide care.
- This is a day/night rotation. When working nights the shift is 7:00 PM to 7:00 AM.
