Job Summary
Responsible for conducting coordination of care activities that support the clinical Service Coordinators in the assessment, service planning, continuous engagement and ongoing evaluation of members receiving Long Term Services and Support (LTSS) benefits. Provides administrative support to the Service Coordinators to ensure all authorizations, referrals, phone calls, and reports are addressed and completed in a timely manner based on regulatory requirements (i.e. Health and Human Services Commission (HHSC)). Collaborates with the member and the participants of the member’s interdisciplinary team (IDT) to ensure medically necessary covered services are provided to the member based on their approved level of care. Engages in consistent communication with members, families, service providers, and regulatory agencies to ensure compliance with program requirements specific to the services and needs outlined in the person-centered care plan.

Work Experience
Three years of healthcare experience required, preferably in a managed care environment in related area of responsibility (i.e. long-term care, home health, hospice, public health or assisted living). Knowledge of Texas Medicaid and health plan processes preferred. Knowledge of medical terminology preferred. Experience as a case management support Associate, in a medical office or health plan is preferred. Strong knowledge of the MS Office suite (Microsoft Excel, Word and PowerPoint programs) is required.
License/Registration/Certification
Community Health Worker (CHW) certification is required within one year of being in the position.
Education and Training
High school diploma or equivalent is required. Associate degree is preferred.
