Your Role on the Team
Position Summary:
The Discharge Coordinator is a professional person who plans, manages and coordinates the discharge plan of the acutely hospitalized patient. The discharge Coordinator works in conjunction with the multidisciplinary team collaborating with the appropriate team members along the continuum of care. The Discharge Coordinator is knowledgeable of available community resources and the healthcare benefits of the acute patient. The Discharge Coordinator will have excellent communication skills both written and verbal and be able to work in an intense environment.

Required Qualifications:
- Must possess the skill, knowledge and ability essential to the successful performance of assigned duties
- Must demonstrate customer service skills appropriate to the job
- Excellent written and verbal communication skills in English
- Ability to maintain a work pace appropriate to the workload
- Ability to establish and maintain effective working relationships across the Health System
Mandatory. Previous healthcare/hospital Discharge Planning experience. Preferred: One year acute care hospital discharge planning experience; Licensure: Current Valid California LVN or RN License, Or Education: Bachelors of Science or Bachelors in Healthcare administration, Or above. Working Knowledge of Medicare/CMS guidelines, Erickson’s Life Skills (age competencies), TJC standards, and NCQA standards. Physical Requirements: Must have dexterity, coordination, visual/auditory acuity to perform all job functions. Can sit and walk for long periods of time; Must be able to function in a busy and distracting environment.
