Your Role on the Team
Position Summary:
Incumbent is responsible for a high-level assessment and reassessment process in the Transfer Center for patient/family who desire to Transfer to UCI Health by setting priorities and planning, organizing, and implementing the transfer of care that is goal-directed toward successful transfer to UCI Health. Collaborates with an interdisciplinary team to facilitate, promote, and coordinate the high quality, individualized, safe and cost-effective patient care to UCI Health. Advocates for patient self-determination and choice. Practices cultural competence in evaluation, planning with awareness, and respect for patient and family diversity. Monitors and coordinates resource utilization throughout the health system and evaluates services needed or being provided by the sending facility. Performs the appropriate utilization review functions utilizing medical staff-approved decision support criteria (InterQual Intensity of Service, Severity of Illness criteria). Concurrently intervenes to enhance patient quality outcomes where appropriate. Reviews clinical information in the medical record reflecting the patient’s true severity of illness through interfacing with the patient’s care team or physicians to facilitate potential patient transfer.

Required Qualifications:
- Strong knowledge of Joint Commission, CMS, CDPH and CCS requirements
- Must possess the skill, knowledge and ability essential to the successful performance of assigned duties
- Must possess complete understanding of acuity levels for specific patient population especially ICU
- Must demonstrate customer service skills appropriate to the job
- Minimum of two years of nursing experience in ICU, ED, Tele, or a step-down unit
- Minimum of 2 years work experience in Case Management
- Excellent written and verbal English communication skills
- All external candidates must have a Bachelor of Science in Nursing
- Advanced computer literacy and proficiency in MS Office Products (Word, Excel, PowerPoint, Access) with the ability to run various reports
- Ability to manage multiple concurrent projects and maintain a work pace appropriate to the workload
- Ability to facilitate and lead interdisciplinary communications among patient care providers
- Ability to establish and maintain effective working relationships across the Health System
Preferred Qualifications:
- Prior Transfer Center Experience
- Knowledge of University policies and procedures
- Experience with Epic
- Charge Nurse/House Supervision, Flight Nursing, or Case Management
- CCM, ACM or Case Management credentialing
- Bilingual skills to communicate effectively with patients and families
Licenses and Certifications:
- RN
- BLS
- ACLS
