Position Custom Scope
UCR Health has a career opportunity for a dedicated Nurse Practitioner with a passion for providing patient centric healthcare to our pediatric patients. The position's primary work location will be at La Quinta outpatient Clinic. However, it will require flexibility to work at other locations as per clinical/operational needs. This position will work independently and in conjunction with attending physicians to provide clinical expertise and care to complex patients by taking detailed medical histories, performing exams, interpreting test results, making diagnoses, referrals, and implement ambulatory treatment. Provides patient/family member education promoting health maintenance, well child assessments, patient and parent education, and patient safety in a pediatrics setting. Provides care to patients from newborn to 21 years of age. Collaborates with others in the clinic to assure smooth flow of patient care consistent with established quality indicators and participates in the overall coordination of patient care. Works closely with MAs and provides back-office guidance. The care delivered will be based on evidence-based and guideline-based practice, and scientific knowledge, and in accordance with specialty standards and requirements set forth by the California Board of Registered Nursing.
Key Responsibilities
Direct Patient Care:
1.Obtain thorough histories and perform complete physical examinations, including ordering, interpreting, and evaluating tests and examinations. Perform well child visits. Order immunizations per the AAP (American Academy of Pediatrics) guidelines. 2. Assume ongoing responsibility for each patient. 3. Diagnose and treat both chronic and episodic disorders including complications of otherwise normal processes. Referrals to other physicians or treatment options.4. Establish and document the health care plan and diagnosis, maintain records of each case that are sufficiently complete for any provider or physician reviewing them to evaluate the effectiveness of the plan. 5. Monitor and evaluate patients in the ambulatory setting with new or ongoing issues. Facilitate transitions of care and care coordination. May communicate with hospitalists and other physicians on patient care. 6. Review and respond to lab/test results, patient Inbasket/MyChart messages, staff messages, refill requests, telephone calls, and virtual patient encounters. 7. Follow department/clinic guidelines for providing continuity of care in collaboration and coordination with physician. 8. Complete all patient care and encounter documentation in the electronic health record system promptly per policy. 9. Coordinate/recommend follow up with appropriate laboratory tests, imaging and diagnostic studies, and consults/referrals. 10. Provide care to achieve quality, safety, efficiency, and patient experience goals. Incumbent will demonstrate a caring, sensitive, and service approach in all interactions with patients, families, and the health care team; 11. Demonstrates proficiency in the use of EHR, computers, and telecommunication systems; consistently demonstrates the knowledge, skills, abilities, and behaviors necessary to provide superior and culturally sensitive service to each other, to our patients, and to, contracted providers, and vendors.
Provides patient/family member education promoting health maintenance, diabetes education, condition-based education including education on minimizing stress, maximizing wellness, encouraging health lifestyle changes, and identifying resources. Screen, address, and counsel on social drivers of health. Educates patients, parents, and caregivers on vaccines and addresses vaccine questions.
Develops and provides educational materials and clinical instructions to other staff providing direct patient care. Serves as a lead for staff to address patient care issues and concerns.
Education & Experience
Degree Requirement:
- Education acceptable by the California Board of Nursing to obtain required licensure.
Experience Requirements:
- A minimum of two years' experience as a Nurse Practitioner.
- Must meet UC credentialing requirements.
- Patient care experience in a pediatrics or primary care setting.
License Requirements:
- Current California Registered Nurse and Nurse Practitioner license.
- Current unrestricted DEA license and National Provider Identifier (NPI) number.
Certification Requirements:
- Current Basic Life Support (BLS) certification.
- Current pediatric advanced life support (PALS) and Neonatal Resuscitation Program (NRP) Or acquire certification within three months of start
- As required for provider Medicare enrollment, national certification from one of the organizations listed below that is recognized as national certifying body
- American Academy of Nurse Practitioners (AANP), American Nurses Credentialing Center, National Certification Corporation for Obstetric, Gynecologic and Neonatal Nursing Specialties, Pediatric Nursing Certification Board (previously named the National Certification Board of Pediatric Nurse Practitioners and Nurses), Oncology Nurses Certification Corporation, AACN Certification Corporation, or National Board on Certification of Hospice and Palliative Nurses.
Knowledge/Skill/Abilities Required:
- Strong interpersonal skills and excellent bedside manner.
- Ability to make emergency decisions quickly.
