Job Summary

This Population Health Project Manager is part of a dynamic team within the Office of Population Health Clinical Programs. This position reports to the Director Clinical Programs.

The Project Manager is a flexible role that requires at least 20% on-site at variable UCSF locations in San Francisco and 80% remote. The incumbent should be able to provide a safe and private home office environment for conducting patient outreach and UCSF business.

UCSF Office of Clinical Programs is developing innovations to support population health as part of the Accountable Care Organization. In order to succeed in these new arenas, we are developing new partnerships and care delivery models for all age groups, which need strong project management, leadership and support. The programs developed in population health clinical programs help achieve the "quadruple aim" of high value care : they provide higher quality, lower cost, and patient centered care across the full care continuum. Examples of the type of work in need of project management include leading the implementation of new patient engagement and health equity strategies focused on preventive care, chronic disease management, and community partnerships.

Given the type of work and our organizational culture, the Project Manager will need to be able to collect and interpret data from various sources, have strong problem solving and presentation skills, have a good understanding of clinical care and be a strong team player. They will serve as the liaison to external healthcare partners as well as key UCSF physician leaders and clinic directors.

Given the early stage of this area of work, the Project Manager must be capable of developing new, feasible ideas to improve health and reduce costs for these targeted populations and able to take on new projects as this area of work continues to rapidly evolve. The Project Manager will provide project management support for division-wide initiatives and dissemination of Population Health innovative care models, as needed.

Department Description

The Office of Population Health (OPH) leads UCSF's development of innovative care models to improve the value and quality of care and improve health outcomes for identified populations and facilitates implementation of these care models across the health system. OPH is responsible for fulfilling UCSF Health's accountable care contracts and fostering care partnerships across a clinical network.

This department is comprised of four main teams : (1) Clinical Programs, (2) Population Health Quality and Performance Improvement, 3) Ambulatory Risk Capture / Medicare Advantage Operations, and 4) Population Health Administration.

Required Qualifications

  • Bachelor's degree in healthcare or a relevant profession, or equivalent experience / training.
  • Two (2) years recent experience in health system administration, ambulatory, primary or specialty care.
  • Highly proficient in multiple software applications (Word, Excel, and PowerPoint, Access, Trello) and database applications. High level of proficiency in Microsoft Excel with the ability to run analytical reports and summarize information.
  • In-depth knowledge of medical terminology and patient health information.
  • Demonstrated capacity to anticipate location specific obstacles and plan for contingencies.
  • Demonstrated attention to detail. Demonstrated ability to determine relevance and prioritize tasks.
  • Demonstrated advanced organization and project management skills, with the ability to lead a team, prioritize tasks, and see projects through from inception to completion on schedule.
  • Demonstrated advanced effective writing skills. Advanced knowledge of all aspects of communications. Demonstrated ability to effectively communicate with all levels of an organization.

Preferred Qualifications

  • California RN Licensure
  • Master's Degree in health care administration field, nursing or other health field and a minimum of 2 years recent experience in health system administration, ambulatory, primary or specialty care
  • Requires advanced knowledge of project management principles and performance improvement concepts.
  • In-depth knowledge of population health, NCQA, CMS and / or HEDIS quality standards, and accountable care or value-based care principles. Broad knowledge of initiatives and an understanding of impact to the organization of such initiatives.
