The Case Manager Role
The Case Manager is a clinician who, in conjunction with other members of the healthcare team, is accountable for timely coordination of quality healthcare services to meet the patient's healthcare needs across the continuum of care.
This position requires knowledge of the development stages as services are provided to infants, children, adolescents, pediatric, adult, and geriatric populations.
Additionally, it requires knowledge of cultural diversity and influences in the response to illness, given the diverse population being served.
Responsibilities
- Initiates and implements utilization/resource management, continued stay monitoring, discharge planning, supporting optimal patient care outcomes along the continuum of care.
Requirements
- Working knowledge of Medicare, Medi-Cal, NCQA, Heidis, and other criteria identified by the Quality/Case Management Department.
- Minimum of 1 year discharge planning experience.
- Working knowledge of managed care and capitation operations.
- Valid, current California Nursing License.
- Must possess the skill, knowledge, and ability essential to the successful performance of assigned duties.
- Excellent written and verbal communication skills in English.
- Ability to establish and maintain effective working relationships across the Health System.
- Must be able to sight and hear normal phone conversation.
- Must have sufficient manual dexterity to utilize computer and typewriter.
- Knowledge of University and medical center organizations, policies, procedures, and forms.
