Job Description
Be a part of a world-class academic healthcare system at UChicago Medicine Ingalls Memorial Hospital as a Registered Nurse Clinical Liaison in the Inpatient Rehab department. Here you will know, follow, and actively support the philosophy, mission and vision of Ingalls Memorial Hospital. You will adhere to all hospital policies, procedures and standard of care. This position is a 100% onsite opportunity . You will need to be based in the greater Chicagoland area.
Under the general supervision of the Nurse Manager, the Clinical Liaison shall be responsible for the completion of the entire pre-admission process, including the development and maintenance of customer relations with internal and external referral sources. The Clinical Liaison shall also demonstrate accountability for and contribution to program development, quality improvement, problem solving and productivity enhancement in a flexible interdisciplinary fashion.
Essential Job Functions
- Completes the pre-admission screening on request from referral sources, according to department policy
- Determines patient’s eligibility for the program
- Offers alternate placement suggestions if needed
- Completes the verification of admission pre-certification and insurance benefits if patient is to be admitted
- Coordinates bed availability and placement
- Notifies nurse manager, therapy manager, scheduler and unit of potential admissions
- Notifies appropriate staff of any special patient needs anticipated (e.g., interpreter, special equipment, discharge planning)
- Instructs patients/family about goals, objectives, philosophy, policies and expectations of the Rehab Program
- Obtains consent for admission from patient/family, and notifies them of admission arrangements
- Develops network with case managers for referrals and as a resource to aid in discharge planning
- Attends discharge planning rounds for appropriate med-surg areas
- Communicates with hospital case managers daily regarding potential post-acute needs of acute patients
- Participates in survey preparations activities
- Reviews in-house census daily for potential referrals
- Functions as a resource for and provide team members with in-service/cross-training on admissions knowledge and skill areas
- Orients other staff to the admissions functions
- Contributes to the development of improved efficiency and productivity of unit clinical/administrative functions
Required Qualifications
- Graduation from an accredited school of nursing with a Bachelor's of Science in Nursing (BSN) OR Associate's (ADN)
- Must hold a current RN Licensure State of Illinois Department of Professional Regulation or valid temporary permit to practice nursing in the State of Illinois
- Active BLS certification from the American Heart Association (AHA)

- One or more years of experience in rehabilitation

Compensation & Benefits Overview
UChicago Medicine is committed to transparency in compensation and benefits. The pay range provided reflects the anticipated wage or salary reasonably expected to be offered for the position.
The pay range is based on a full-time equivalent (1.0 FTE) and is reflective of current market data, reviewed on an annual basis. Compensation offered at the time of hire will vary based on candidate qualifications and experience and organizational considerations, such as internal equity. Pay ranges for employees subject to Collective Bargaining Agreements are negotiated by the medical center and their respective union.
