UConn Health is looking for a highly skilled Palliative Care APRN with a passion for patient experience. This position is accountable for independently performing a full range of tasks in delivery of direct clinical services. This position will work as a key member of the Palliative Medicine consultation service in the John Dempsey Hospital at UConn Health in Farmington, CT. The ideal candidate will be certified as an Advanced Certified Hospice & Palliative Nurse. The candidate will work in collaboration with members of the interdisciplinary care team to deliver expert palliative care services to patients and their families living with chronic disease, serious illness or acute life-threatening illness.
SCHEDULE: This is a per diem, as needed position with no benefits
HOURLY RATE: $100.00
This crucial role is responsible for independently performing a full range of tasks in delivery of direct clinical care services involving medical therapeutics to seriously ill patients.
MINIMUM QUALIFICATIONS, KNOWLEDGE, SKILLS, ABILITIES:
Considerable knowledge of anatomy and physiology

EXPERIENCE AND TRAINING:
GENERAL EXPERIENCE:
Certification as an Advanced Practice Nurse from an accredited Nurse Practitioner program
Incumbents in this class must possess a valid license to practice in the State of Connecticut
SPECIAL REQUIREMENT:
ACHPN certification (APRN Palliative Care and Hospice Certification)
Specialty palliative care experience in an inpatient hospital setting
EXAMPLES OF DUTIES:
Assumes direct patient care responsibilities for a group of patients
Performs a comprehensive palliative care patient assessment for the diagnosis, treatment, and management of acute or chronic serious illness
Conducts a comprehensive assessment of the patient and family to promote informed decision making, goals of care and advanced care planning
Utilizes expertise in pain and symptom management to improve quality of life and symptom burden
Identifies and implements medical plan of care in collaboration with medical team
Collaborates with members of the interdisciplinary health care team to coordinate plans of care unique to patient and family
Utilize expert communications skills to lead goals of care conversations with patient/family, assist with healthcare decision making and conduct advanced care planning discussions
Work directly with program leadership to ensure all palliative care standards are met
Maintains accurate patient medical records
Participate in the development and maintenance of clinical practice guidelines, policies & procedures
May participate in palliative care clinical research
Provides patient counseling and education
Provides palliative care education and training to providers, staff and students
Participates in palliative care quality improvement initiatives
Contributes to the development and growth of palliative care services to align with organizational vision and mission
