Minimum Education and Experience Requirements: Bachelors in Physical or Occupational Therapy. Five years clinical experience with a minimum of three years in a management/supervisory capacity in a health care setting. Motor Vehicle Operator Designation: None provided Licensure/Certification/Registration: Licensed Physical or Occupational Therapist in the State of Florida.
