Overview
Responsible for identifying patients appropriate for post acute home care services and providing information and education on a variety of post acute care services available in the continuum of care. Develops relationships with the internal health care team, as well as homecare agencies, and serves as a link between hospital, physician, community physicians, and home health agency. Contributes to the development and growth of Shands HealthCare.Qualifications
Minimum Education and Experience Requirements: Bachelor's degree in nursing, RN, or LPN; prefer registered nurse with current Florida license. Minimum of three years healthcare experience, including one year in homecare. Experience in homecare, case management, coordinating patient care preferred. Knowledge of JCAHO, federal, state, local and agency policies relevant to homecare and/or case management, in addition to full understanding of all services and programs offered by the hospital. Knowledge of Medicare reimbursement guidelines/issues and other payer requirements; ability to establish and maintain positive working relationships with physicians and other healthcare personnel. Excellent verbal and written communication skills required to present homecare services to physicians and to serve as an educational resource and trainer to hospital staff. Motor Vehicle Operator Designation: Employees in this position: Will not operate vehicles for an assigned business purpose NOTE: A frequent driver is defined as one who uses his/her personal or Shands automobile a) at least once daily, b) at least five individual trips per week or c) drives, on average, over 150 miles per week in the performance of his/her job.
