Salary: The budgeted salary range for the position is $118,728-150,291.

Position Summary

The Advanced Practice Provider (APP) provides comprehensive health care for pre, and post organ transplant patients as determined by scope of practice guidelines and specialty. This includes completion of a health assessment, determining diagnosis, planning and prescribing treatment within the parameters of medical staff privileges at UI Health and area of expertise. The APP participates in the educational experiences of nursing students, medical students, residents and other health care disciplines An APP is a registered APRN or PA-C and holds a master's or doctoral degree. The APP functions as part of the multidisciplinary health team in accordance with privileges approved by the credentialing committee to provide high-quality, cost-effective advanced care to patients within the APP's scope of practice. The APP is required to have and maintain a faculty appointment to receive clinical privileges to practice at the University of Illinois Health System.

Duties & Responsibilities

  • Perform and document a complete history and physical examination on-site at transplant outreach clinics for pre and post-transplant patients.
  • Perform routine clinical procedures independently as outlined in privileges granted. These procedures include but not limited to: suture/staples, accuchecks, frailty assessment.
  • Coordinates care for transplant recipients from referral and /or until waitlisted, all potential recipients with living donors and all living donor patients.
  • Ensures that all pre/post-transplant patient coordination and patient education is documented and is following UNOS, CMS transplant program guidelines and UI Health policies/procedures.
  • In collaboration with the provider, orders all testing for evaluation as needed and reviews results with the provider in a timely manner following UNOS and CMS guidelines.
  • Develops, coordinates, and communicates the patient's plan of care based on ongoing assessment of the patient and input from the patient's family.
  • Uses clinical expertise to collaborate with multi-disciplinary health care team to improve patient outcomes.
  • Consults with and implements orders by performing interventions as prescribed by the provider and documenting accurately in the electronic medical record.
  • Assesses, engages and obtains clinical information from multi-disciplinary team, patient and/or family to develop an appropriate patient educational plan.
  • Recognizes symptoms, establishes priorities and responds to patients changing needs throughout the transplant care continuum.
  • Ensures that the patient is added to the UNOS waiting list and the required listing letters are sent in a timely manner per UNOS requirements.
  • Coordinates and schedules all living donor transplant surgeries as assigned.
  • Attends and participates in transplant QAPI meetings, listing meetings, department meetings, multidisciplinary planning meetings and other meetings as needed.
  • Provides cross coverage for the transplant data coordinator by entering living donor information in UNOS prior to the living donor surgery as assigned.
  • Leads, coordinates and delivers pre/post patient care, research and evidence-based practice through participant selection, coordination of the protocol(s), data collection, and other associated activities.
  • Demonstrates accountability for professional conduct.
  • Engages nursing and support personnel by delegating appropriate tasks per the Illinois State Nurse Practice Act and provides, support and feedback.
  • Demonstrates a commitment to UI Health's values through teamwork and effective communication in an effort to achieve quality outcomes, patient safety, customer satisfaction and a clean, safe and therapeutic environment.
  • Integrates theoretical knowledge, evidence-based research, current standards of practice and clinical experience to provide “quality" patient care as measured by established nursing quality stand.

Minimum Qualifications

  • Master’s Degree in Nursing, Doctorate of Nursing Practice, or Physician Assistant Certification required.
  • Graduation from accredited APRN or Physician Assistant program required
  • Current license as a Professional Registered Nurse, Advanced Practice or Physician Assistant required.
  • Registered RN or PA in the State of Illinois required.
  • Current IL Controlled Substance License required
  • Ability to meet all requirements for medical staff privileges required

Preferred Qualifications

  • Effective communication skills (oral and written) that facilitate therapeutic relationships between physicians, APRN and PA providers as well as multi-disciplinary care team.
  • Demonstrated experience in leadership, multidisciplinary collaboration, interpersonal communication, education, clinical experience.
  • Experience in a Transplant department and/or Transplant Quality Team preferred.
  • Knowledge of Transplant Quality regulations preferred.
