Staff Nurse - CDD (70%) day shift

Provide patient care to infants, children and young adults requiring specialized care for chronic conditions as they relate to developmental disabilities, including mental health and behavioral health conditions. As a member of an interdisciplinary healthcare team, this position will coordinate care of patients/families being served in the clinic, provide triage services and monitor overall clinic flow and processes. Assess patients' nursing care needs, evaluate effectiveness of care provided, and integrate nursing theory and research into clinical practice. Provide leadership to members of the nursing team in order to maintain patient care standards. Candidates should be able to support and manage change, work as a member of a health care team and work in a fast-paced environment.

*Shift will occur between the hours of 7:30 a.m. to 5:30 p.m. Monday-Friday.

*Percent of time: 70%

  • Applies nursing theory to the assessment, diagnosis, treatment, and evaluation of patient responses to health and illness.
  • Assesses patient health care preferences and expectations.
  • Maintains responsibility for quality patient care and ensure patient safety while collaborating the continuum of care across settings.
  • Develops patient care plan based on patient condition, age, and psychological, educational, and socioeconomic conditions that prescribes interventions to attain expected outcomes.
  • Implements patient care plan according to patient priorities, which includes interventions identified through nursing diagnoses. Documents/reports interventions and patient/family response.
  • Collaborates and integrates contributions of patients and families into multidisciplinary patient care plan.
  • Implements nursing interventions and care to infants, children and adults requiring specialized care for chronic conditions as they relate to developmental disabilities, including mental health conditions following department, division, and/or unit policies, procedures, and protocols to maintain safe environment for patients, visitors and staff. Follows universal precautions.
  • As a member of a multi-disciplinary healthcare team, provide patient care triage, both in person and over the telephone; serve as liaison for patients/families, clinicians, physicians and community resources to assist in requesting needed cares.
  • Coordinate care of the patient/family being served in the clinic, including coordination before, during or after a visit; and assists with monitoring overall clinic flow.

Required Qualifications:

  • An Associate's Degree, Diploma in Nursing, Baccalaureate Degree, Professional Masters of Nursing and Healthcare Practice (MNHP), MSN/Clinical Nurse Leader or a Master's Degree in Nursing (MSN, MA) is required.
  • Current license to practice nursing in Iowa.
  • Excellent written and verbal communication skills are required.
  • Previous nursing experience working with pediatric patients.
  • Experience demonstrating excellence in service with patients/families, nursing staff, and interdisciplinary team members as demonstrated through written and verbal interactions.
  • Experience developing and maintaining collaborative working relationships and working in a team setting.
  • Strong organizational skills including the ability to manage multiple tasks and adhere to deadlines.
  • Excellent attention to detail.
  • Proficiency in computer programs, including MS word and Outlook.

Desired Qualifications:

  • Baccalaureate Degree in Nursing or Master's Degree in Nursing.
  • Experience working in a clinical or hospital setting that includes working with patients and families with chronic conditions, development disabilities and mental health conditions.
  • Experience working with pediatric patients with mental health and behavioral health conditions and their families.
  • One year experience working in a patient care setting that included triage responsibilities.
  • Experience working in a multi/interdisciplinary health care team environment.
  • Knowledge of community programs and services for people with disabilities.
  • Experience in leading process improvement teams.
  • Experience working with an electronic medical record program.
