General Summary
As a Clinical Pharmacy Specialist , you will provide high-level direct patient care for a specialized population, ensuring adherence to professional standards. You will advise and consult with patients, families, and medical staff. Your role includes participating in research and drug use / outcome management programs, publishing data, and presenting at meetings. Additionally, you will instruct and train pharmacy residents and students, oversee clinical services provided by pharmacists, and mentor clinical pharmacists.
Principal Responsibilities and Tasks
Clinical Pharmacy Services
- Participate in direct patient care with medical or surgical teams.
- Assist with therapeutic decisions, including drug selection, dosing, and monitoring.
- Develop patient care plans with the medical team.
- Recommend therapies based on medical literature and clinical experience.
- Provide clear, succinct verbal or written recommendations.
- Consider age, weight, and organ function when evaluating therapies.
- Interview patients and conduct physical assessments to determine medical history and therapy response.
- Interpret laboratory and patient-specific data to develop and monitor treatment plans.
- Apply pharmacokinetic principles for safe, effective, and economical regimens.
- Respond to therapeutic queries from healthcare providers.
- Identify and correct drug-related problems, such as ineffective therapy or adverse reactions.
- Establish procedures to detect and avoid significant drug interactions.
- Perform basic cardiac life support and manage drug therapy during medical emergencies.
- Ensure continuity of care by collaborating with other health professionals and organizations.
- Counsel patients and caregivers on medication administration, potential adverse reactions, and storage.
Drug Use, Outcome and Formulary Management
Qualifications
Education :
Required : Bachelor of Pharmacy from an accredited institution (American Association of Colleges of Pharmacy).
Experience :
Required : Minimum of 5 years of clinical pharmacy experience with a Bachelor’s degree.
Licensure :
Required : Licensure by the Maryland State Board of Pharmacy.
Certifications :
Preferred : Certification from the Board of Pharmaceutical Specialties.
Skills and Abilities :
Proficiency in personal computer operations and applications (e.g., spreadsheets, database management, word processing).
