Community Outreach Worker II-(OK)
The JACQUES Initiative of the Institute of Human Virology (IHV) at the University of Maryland, Baltimore (UMB) is expanding its community outreach program. This Community Outreach Worker II position will identify, link to care, and coordinate services for individuals newly diagnosed as HIV and those who are previously-diagnosed HIV and currently out of care. Encounters will happen in the hospital, as well as in the larger Baltimore Metropolitan Area community.

Essential Functions :
- Coordinate the identification of newly diagnosed or previously-diagnosed HIV individuals.
- Assessment of psychosocial barriers and social determinants of health.
- Conduct outreach activities to provide education and testing for HIV and HCV.
- Provide PrEP navigation services and support ED testing to foster engagement into medical care and case management.
- Build and maintain strong relationships with community members and partners.
- Keep records of program activities : progress, status, and outcomes, and prepare reports for management.
- Performs all the duties of a Community Outreach Worker I.
Qualifications
Education : High-school diploma or GED.
Experience : Two years in community outreach, health care, research, and / or social services.
The ideal candidate should have experience working in the field of HIV / AIDS providing direct services to clients, including psychosocial support services, case management, psychosocial assessment, insurance application, and resources coordination. In addition, familiarity with HIV resources in Baltimore, Ryan White standards of care and documentation, use of electronic medical records, and cultural sensitivity is highly preferred.
OTHER :
Except for qualifications established by law, additional related experience and formal education in which one has gained the knowledge, skills, and abilities required for full performance of the work of the job class may be substituted for the education or experience requirement on a year-for-year basis with 30 college credits being equivalent to one year of experience.
Based on the duties of this position, selected candidates are eligible to participate in the Bargaining Unit (Union).
Salary Range : $42, - $52, (Commensurate with education and experience)
