Job Description
General Purpose of Job:
The purpose of the Sparrow PGY1 Pharmacy Residency Programs is to build on the Doctor of Pharmacy (Pharm.D.) education and outcomes to contribute to the development of clinical pharmacists responsible for medication-related care of patients with a wide range of conditions and to prepare graduate pharmacists for the practice of pharmacy. Upon completion of the PGY1 program, the resident will be eligible for board certification and a postgraduate year two (PGY2) pharmacy residency training program.
Essential Duties:
This job description is intended to cover the minimum essential duties assigned on a regular basis. Caregivers may be asked to perform additional duties as assigned by their leader. Leadership has the right to alter or modify the duties of the position.
- Provides clinical pharmacy services assigned as part of the training program.
- Provides drug therapy monitoring of assigned patients and communicates relevant findings and recommendations to other healthcare professionals who are also responsible for the patient’s care.
- Develops patient-specific drug therapy management plans and provides oral and written consultations to other healthcare professionals regarding drug therapy selection and management in a timely and accurate manner.
- Provides patient education and counseling regarding drug therapy and drug-related disease prevention.
- Monitors, detects, reports, and manages adverse drug events and medication incidents.
- Participates in medication-use evaluations and other quality improvement programs.
- Initiates and participates in drug-related research.
- Provides accurate and comprehensive information about medications and patient-specific medication information to other pharmacists and healthcare professionals.
- Participates in staff development activities.
- Performs as a preceptor for university affiliated pharmacy externship students.
- Performs staff pharmacist functions on a rotational basis as part of the training program.
- Develops, implements, and participates in the revision of medication use policies and procedures.
- Participates in appropriate committees related to medication use. Provides quality patient care by considering the age specific, developmental, and cultural needs through competent clinical practice.
- Demonstrates unit / area competencies.
- Maintains good rapport and cooperative relationships. Approaches conflict in a constructive manner. Helps to identify problems, offer solutions, and participate in their resolution.
- Behaves in accordance with the Mission, Vision and Values of Sparrow Health Systems.
- Assumes responsibility for performance of job duties in the safest possible manner, to assure personal safety and that of coworkers, and to report all preventable hazards and unsafe practices immediately to management.

- Demonstrates knowledge and respects patient, service provider, organizational confidentiality and HIPAA Security procedures and protocols as defined under the HIPAA Privacy and Security Procedures.
- Follows established HIPAA privacy procedures when using and/or disclosing protected health information.
- Maintains and protects patients’ rights under the HIPAA Privacy Standards.
- Compounding and supervising the compounding of Non-sterile medications up to and including Moderate Compounding as defined by USP<795> and Sterile medications up to and including Medium Risk as defined by USP <797>
- Compounding caregivers adhere to the prescription or medical order for the strength, quality and purity of compounding preparations. Compounded products are packaged, labeled and dispensed according to current laws and regulations.
