Job Summary

The Children's Intestinal Rehabilitation Program (ChIRP) at Michigan Medicine represents the best in collaborative care for a complex patient population. The ChIRP team represents the largest and most robust intestinal failure and short bowel program in the state of Michigan. One of the unique aspects of this program is the daily involvement of a dedicated social worker which improves the complex care needed by this patient population.
Children with intestinal failure and short bowel syndrome, as well as their families, experience significant psychosocial and economic challenges, thus their care is at risk for complications and safety needs. Among these are adjustment concerns, school attendance/performance, economic stability, psychological/psychiatric needs, non-adherence to care, and the potential need for frequent admissions, often including surgical interventions. All of these complications and risks are concerns that the MSW role helps to address and assists our families to navigate effectively ? the true embodiment of patient and family centered care.
One of the unique aspects of the ChIRP MSW role is its combination of psychosocial care in the inpatient and outpatient settings for the ChIRP patient population, allowing for important continuity of these children with complex needs.
ChIRP is a multidisciplinary team that provides comprehensive care to infants, children, teenagers and young adults with intestinal failure and short bowel syndrome. This multidisciplinary team consists of experts from pediatric GI, pediatric surgery, nutrition, pharmacy, nursing and social work. ChIRP offers a full spectrum of services, from medical and nutritional management to psychosocial support and surgical services. Research has shown that the outcomes of children with intestinal failure and short bowel syndrome are the best when they are cared for by multidisciplinary teams which include a clinical social worker.

Responsibilities*

You will be responsible for:

  • Evaluation of psychosocial needs and provision of billable social work interventions with a high degree of autonomy
  • Care Management and care coordination, scheduling coordination
  • Navigation of barriers to care
  • Transition support/care continuity
  • Facilitating patient and family participation in appropriate health care decision making
  • Crisis intervention
  • Patient and/or family support and counseling
  • Support group co-facilitation
  • Liaison and referral to community agencies
  • Participation and/or facilitation of multi-disciplinary team meetings, rounds, educational activities, and patient advisory board
  • Referral monitoring and management
  • Working directly with patients both face-to-face and virtually by phone or video visit.

Required Qualifications*

  • A Master's degree in social work from a school accredited by the Council of Social Work Education is necessary.
  • Some experience of professional social work is necessary.
  • Current licensure (LLMSW/LMSW) to practice Social Work in the State of Michigan is required. Licensure will be verified via PSV by department administration.

Desired Qualifications*

  • Previous experience with pediatric medical and mental health social work.
  • Previous experience with families and children around issues of chronic illness, adjustment to illness and disability, learning/school issues, locating community resources
  • Proficiency in adjustment, loss, and bereavement issues
  • Skilled in developing rapport and effectively communicating with a diverse patient population
  • Skilled in utilizing mental health screening tools such as ASQ, C-SSRS, PHQ-9, GAD-7
  • Efficient, accurate assessment of patient needs and effective care planning
  • Effective organizational skills, flexibility, self-motivation, the ability to effectively prioritize multiple competing tasks in a dynamic medical setting
  • Independent, sound decision-making is necessary for success in this position
  • Effective collaboration with multi-disciplinary teams
  • Proficiency in keeping accurate and timely clinical records by computer

Work Schedule

This is a full time, 40 hour/week position. The typical schedule is Monday-Friday with start and end times 8:00-4:30 or 8:30-5:00.

Work Locations

C.S. Mott Hospital, 1540 E. Hospital Drive, Ann Arbor, MI 48109. This role may have the option of being a hybrid position located remotely and at C.S. Mott Hospital, however work location is adjusted as needed to accommodate team needs and position responsibilities. Candidates need to be flexible and willing to adjust between hybrid and onsite work as needed.

Underfill Statement

This position may be underfilled at a lower classification depending on the qualifications of the selected candidate. Advancement through the career ladder at the Expert or Clinical Scholar Level is required for placement in the MSW Senior Classification.

This position is included in the bargaining unit represented by the United Michigan Medicine Allied Professionals (UMMAP), which represents bargaining unit members in all matters with respect to wages, benefits, hours, and other terms and conditions of employment.

Michigan Medicine conducts background screening and pre-employment drug testing on job candidates upon acceptance of a contingent job offer and may use a third party administrator to conduct background screenings.
