Key Responsibilities
- Coordinate all systems/services required for an organized, multidisciplinary, patient-centered care team approach
- Manage the course of treatment of patients, coordinating care with physicians, nurses, and other staff
- Conduct initial and ongoing assessments, initiate disease management protocols, determine and manage outcomes
- Ensure continuity of care through discharge planning, utilization of resources, and analysis of variances
- Function as a contact person for patients, families, healthcare team members, community resources, and employees
Requirements
- Graduate of a nationally accredited nursing program
- 1 year of directly related experience in a hospital setting
- RN licensure in New Mexico or reciprocal agreement
- CPR for Healthcare/BLS provider or professional rescuer certification within 30 days
Work Environment
This role requires working in a fast-paced hospital environment with minor hazards such as physical risks, dirt, dust, fumes, and noise.
