Job Summary
We are seeking an experienced professional to join our Neuropsychology department as a Community Support Worker. In this position, you will work collaboratively with our multidisciplinary team to provide coordinated services and resources to individuals and families, promoting rehabilitation, recovery, and resiliency.
Key Responsibilities
- Assessment and Intervention: Conduct ongoing assessments to identify personal strengths, needs, and barriers to attaining self-identified goals. Develop and implement personalized interventions to support individuals in crisis situations.
- Service Coordination: Assist clients in obtaining access to necessary rehabilitative, medical, and other services, including financial and medical benefits/entitlements.
- Crisis Intervention: Assess, support, and intervene in crisis situations, using natural supports and alternatives to hospital emergency departments and inpatient hospital services.
- Documentation and Tracking: Document all client encounters and maintain current and comprehensive client files, including post-discharge follow-up contact as appropriate.
Requirements
To be successful in this role, you will need:
- A Bachelor's degree in a related field (e.g., social work, psychology, counseling).
- At least 1 year of direct experience working with individuals with mental health conditions.
- CPR for Healthcare/BLS Provider certification within 30 days of hire.
- Ability to work collaboratively as part of a multidisciplinary team.
