Job Description
In this role, you will have the opportunity to work with a talented team of professionals who share your passion for delivering high-quality care. As a Community Support Worker, you will work closely with our multidisciplinary team to provide coordinated services and resources, promoting rehabilitation, recovery, and resiliency.
Key Responsibilities
- Rehabilitation and Recovery: Coordinate and provide services and resources to individuals and families, promoting rehabilitation, recovery, and resiliency.
- Service Planning: Assist in the development and coordination of individual service plans based on identified strengths and goals.
- Resource Coordination: Assist clients in obtaining access to necessary rehabilitative, medical, and other services, including financial and medical benefits/entitlements.
- Crisis Intervention: Assess, support, and intervene in crisis situations, using natural supports and alternatives to hospital emergency departments and inpatient hospital services.
Requirements
To be successful in this role, you will need:
- A Bachelor's degree in a related field (e.g., social work, psychology, counseling).
- At least 1 year of direct experience working with individuals with mental health conditions.
- CPR for Healthcare/BLS Provider certification within 30 days of hire.
- Ability to work collaboratively as part of a multidisciplinary team.
