Job Description
- Develops, reviews, and evaluates clinical guidelines and Drug Utilization Reviews (DUR), provides consultations, develops programs, prepares articles, and conducts research.
Duties:
- Provides consultations, performs drug utilization review/medication reconciliation, contributes to education, contributes to clinic operations and process development, develops programs, prepares articles, and contributes to research.
- Provides clinical consults and collaborative patient care for complex cases to address medication-related problems and promote safe and effective therapy in collaboration with providers.
- Recommends, implements, monitors and/or adjusts therapy with appropriate documentation in accordance with collaborative agreements and/or Oklahoma Pharmacy Law.
- Performs prospective and retrospective Drug Utilization Review (DUR) and medical record review to reconcile medication profiles from inpatient and outpatient settings.
- Coordinates and contributes to orientation, supervision and evaluation of pharmacy students and pharmacy residents in the delivery of patient care and education.
- Contributes to clinic operations/process development and interfaces with personnel in support of service initiatives.
- Participates in the development and presentation of service initiatives.
- Prepares or reviews articles pertaining to service initiatives and outcomes.
- Contributes to research and reviews of the literature to document service initiatives and outcomes.
- Performs various duties as needed to successfully fulfill the function of the position.
Required education: Bachelor’s Degree in Pharmacy or PharmD, AND:
- 12 months Pharmacy experience
Certifications:
- Licensed Pharmacist by the Oklahoma State Board of Pharmacy or obtain licensure within 90 days of hire.
- Must possess or be able to obtain an Oklahoma Preceptor’s license by the Oklahoma State Board of Pharmacy within one year of hire.
Skills:
- Provide consultations commensurate with knowledge, skills and abilities obtained from PGY1 pharmacy residency with PGY2 ambulatory care residency or equivalent experience.
- Capacity to complete Board Certification in Pharmacotherapy or Ambulatory Care Pharmacy required within one year of hire.
Working Conditions:
- Physical: Sitting for long periods of time. Reading. Manual dexterity.
- Environmental: Office Environment
Department Preferences:
- Preferred: PharmD
- PGY1 pharmacy residency with PGY2 ambulatory care residency or equivalent
