Under general direction and with considerable independence, performs case management duties with patients connected to CCBHC clinics with the goal of connecting and enrolling these patients into Health Home Care Management. Establishes and maintains cooperative working relationships with community providers and resources to obtain needed services and support for enrolled clinic patients. Works with patients up to and including the Health Home Care Management enrollment process and facilitates warm hand
off of services to an assigned Health Home Care Manager.

ESSENTIONAL DUTIES
- Conducts assessment of identified patients for Targeted case management services, immediate community referrals, and Health Home Care Management services. Completes and maintains professional documentation in compliance with all URMC, OMH, and DOH regulations for the provision of Targeted Case Management and Health Home services. Assesses successful completion of goals for Targeted Case Management services. Provides Targeted Case Management services as indicated to ensure access to healthcare and meeting immediate needs of patients. Available to assist clinicians as needed to support case management needs. Available to triage case management needs for CCBHC program recipients and Health Home Outreach patients on an urgent basis including walk-ins, contributing to the mission and vision of the CCBHC.
- Coordinates Outreach and engagement activities focused on finding, connecting and retaining patients in appropriate community services. Mobilizes family and natural support networks when necessary. Provides milieu management in reception areas, supporting Health Home patients and enrolled Targeted Case Management patients with immediate needs and service supports.
- Completes Health Home Care Management referrals, community agency referrals, and self-referrals for CCBHC patients who are not yet connected to Health Home Care Management services.
- Coordinates outreach and engagement activities focused on finding, connecting and retaining patients in Health Home Care Management Services with a primary focus on patients connected to Targeted Case Management through Certified Community Behavioral Health Clinics. Interacts with patients via telephonic outreach and in person encounters, such as primary care settings, behavioral health clinics, home, jail, hospital, homeless shelters, and other community settings. Responsible for completing all paperwork and documentation associated with enrolling a patient into Health Home Care Management services. Once patient is enrolled in Health Home Care Management, responsible for facilitating a warm hand off to the assigned Health Home Care Coordinator.
Qualifications
- Bachelor’s Degree in an appropriate human services field required.
- One year of experience in providing direct services to people with serious mental illness, intellectual/developmental disabilities, alcoholism/substance abuse, or experience effectively linking people
- with services that address social determinants of health or an equivalent combination of education and experience required.
- Must possess and maintain a valid New York State driver’s license, have a satisfactory driving record and have access to an automobile.
- Must pass NYS DOH Health Home and URMC background check requirements.
