Position Summary:
Provides quality patient-oriented pharmaceutical care which includes providing drug information and consultation to physicians and other health care personnel, receiving medication orders written by physicians, reviewing each for appropriate therapy, managing the dispensing of medications to the patient care areas and monitoring patient response to drug therapy.
Documents activities in the medical record
Hospital policies and procedures are followed and documented including that entries are signed, dated, timed, legible, and concise.
Activities and recommended plans are clearly presented.
Manages drug distribution to patients/patient care areas
Distribution activities conform to standards of practice, appropriate safety practices, and hospital policies and procedures.
Ensures that patients receive the correct medication in a timely manner.
Reviews and interprets medication orders
Orders are reviewed/validated in a timely manner and consistent with established guidelines.
Therapy and dosage is appropriate for diagnosis.
Allergy conflicts, drug interactions, and incompatibilities are identified and resolved.
Manages drug therapy
Drug therapy problems are identified and problem lists are complete.
Drug therapy regimens/recommendations are designed to reflect best evidence.
Appropriate monitoring plans are developed and implemented.
Recommendations are clearly presented.
Protocols are managed in accordance with approved guidelines.
Responds to consults and provides expert recommendations
Responses are timely and occur in accordance with established guidelines.
Responses are accurate, complete, and presented in a professional manner.
Position Qualification:
Doctor of pharmacy from an ACPE Accredited College of Pharmacy and a Pharmacy Residency from an ASHP accredited residency program;
Licensure as a Pharmacist in the State of Tennessee is required.
