JOB SUMMARY
The GI/Irritable Bowel Disease Coordinator works closely with physicians in Gastroenterology and other specialty areas to coordinate a patient’s plan of care, manage prior authorizations, coordinate infusions, coordinate post-hospital care and retrieving documents needed to support care. Position provides expert individualized care coordination to patients and their families ensuring a timely schedule for diagnostic work-up and medication starts. Position also communicates between the patient and the multidisciplinary team ensuring the patient received all the necessary referrals and follow-up.
MAJOR RESPONSIBILITIES/DUTIES/CRITICAL TASKS
- Maintains knowledge of current state, federal, and facility requirements to meet current healthcare goals related to patient safety, reimbursement, and healthcare quality standards.
- Works with a multidisciplinary team to develop and implement an up-to-date plan of care.
- Assesses, develops, and implements effective patient/family teaching and accurately assesses readiness to learn.
- Communicates with all members of the healthcare team on behalf of the patient.
- Collaborates with marketing and outreach departments to educate referring physicians and communities about facilities and services offered.
- Maintains a patient database and participates in patient recruitment for clinical research.
- Facilitate and keep track of patient access to IBD therapy, including managing prior authorizations, coordinating infusions, and following up with patients to ensure they have received their therapy.
- Coordinating post-hospital care for recently admitted IBD patients, including setting up follow up, ensuring patient has obtained medications, and patient education.
- Assistance with patient-requested documentation to support IBD care and disability.
- Teaching patients how to use injectable medications.
- Follow up on patients who are no shows at clinic follow up or endoscopy to reschedule appointment.
- Organize educational materials and handouts to give to patient’s post-clinic appointment.
- In the future: IBD patient registry by enrolling newly diagnosed patient. Working with the research coordinators and clinic staff to enroll IBD patients in research trials.
- Other duties as assigned.
- Answer to a direct hotline for patients who may be having a flare or have urgent concerns and help triage these patients to see if they need to be seen in clinic urgently.
- Proactively check on high risk patients (noncompliant, severe disease, started on new therapy, difficult social situation, etc.).

MINIMUM REQUIRED EDUCATION/EXPERIENCE
