What are the responsibilities and job description for the Nurse Navigator- Neurology Clinic position at University of Texas at Tyler Health Science Center?
JOB SUMMARY:
The Neurology Nurse Navigator works closely with physicians in various neurologic specialty areas and coordinates a patient’s plan of care, communication, and program. Position provides expert individualized care coordination to patients and their families ensuring a timely schedule for diagnostic work-up and treatment of various neurological problems. Position also communicates between the patient and the multidisciplinary team ensuring the patient received all the necessary referrals, authorizations, and follow-up.
MAJOR RESPONSIBILITIES/DUTIES/CRITICAL TASKS:
- Develops and enhances processes with the aim to improve the clinical experience for patients and staff.
- Works closely with Information Technology and Quality Departments to ensure quality measures are met and data is recorded accurately.
- Utilizes clinical protocols to refer patients for appropriate screening and diagnostic testing.
- Maintains knowledge of current state, federal, and facility requirements to meet current healthcare goals related to patient safety, reimbursement, and healthcare quality standards.
- Works with a multidisciplinary team to develop and implement an up-to-date plan of care.
- Assesses, develops, and implements effective patient/family teaching and accurately assesses readiness to learn.
- Communicates with all members of the healthcare team on behalf of the patient o Assists the patient with referrals to the patient financial counselor when needed.
- Collaborates with marketing and outreach departments to educate referring physicians and communities about facilities and services offered.
- Works with Residents and Fellows on quality improvement projects.
- Maintains a patient database and participates in patient recruitment for clinical research.
- Other duties as assigned

MINIMUM REQUIRED EDUCATION/EXPERIENCE:
- Graduate from an accredited school of Nursing.
- Must be a licensed RN.
- Must possess and maintain a current license or valid permit to practice in Texas.
- Must possess and maintain current CPR Healthcare Provider.
REQUIRED LICENSES/ CERTIFICATION:
- Must possess and maintain a current license or valid permit to practice in Texas.
- Must possess and maintain current BLS certification.
