ESSENTIAL JOB FUNCTIONS AND ACCOUNTABILITIES
- Assist in providing primary and acute medical care under supervision of Physician and APP
- Obtain comprehensive and systematic health history, including physical, social, and emotional components
- Assist patients in understanding diagnosis, treatment options, and resources available, including educating eligible patients about appropriate clinical research studies and technologies
- Perform physical examination or focused evaluation and present to Physician or APP
- Request physician consultation when a health problem calls for knowledge or skill outside of authorized level of care
- Develop, implement, and evaluate individualized plans of care based on history, physical assessment, and patient consultation
- Arrange for appropriate follow up. Serve as an essential link between patients and all other care providers. Facilitate appointments 9imaging, procedures, therapies, etc.) and consult with multi-disciplinary team
- Performs phone triage accurately by involving Physicians as needed while using critical thinking skills and advanced knowledge to handle most patient care issues.
- Assist with the development of clinical procedures or patient assessment and management plans, utilizing current medical and nursing resources. Track and monitors patients progress; consults with physicians, APP, designee, or other members of the care team as needed; refers to appropriate Physician or program when instructed by Physician
- Evaluate needs of individuals for community and/or University referral resources and make appropriate referrals
- Collaborate with other health professionals in planning and ensuring continuity of care
- Assist in identifying and planning for needed health promotion and disease prevention programs
- Act as a resource person to provide health information and/or presentations
- Comply with institutional, federal, state, acute care settings, nursing facility, or LTACH regulatory standards in the delivery of patient care
- Maintain knowledge and skills at a current, optimum level and demonstrate a commitment to professional development by participating in professional activities and educational programs and continuing education through independent reading and attending continuing education programs
- Training in compassionate and knowledgeable interaction aimed at meeting needs of patient/family
- Complete medical records in a timely fashion according to the byelaws of UTMC
- Participate in performance improvement projects. Attends team and staff meetings as scheduled
- Provide excellent customer service to both internal and external customer, keeping the patient the number one priority
- Undergo regular peer review with the results being utilized to address any identified performance issues. Peer review results may also be used during annual performance evaluations
- Maintain legible, accurate, and confidential medical records. Document all medical evaluations, diagnoses, procedures, treatments, outcomes, education, referrals, and consultations consistent with National Committee for Quality Assurance, Join Commission Accreditation of Healthcare Organizations, Center for Medicaid Services, and state regulatory standards
- Assimilate new knowledge gained from conferences, workshops, professional literature, or “hands-on training” into clinical practice
- Other duties as assigned
REQUIRED QUALIFICATIONS
- Education: Associates Degree in Nursing, Diploma in Nursing acceptable

License and/or Certification:
- Active nursing License (Registered Nurse) in the state of Ohio
- BLS Certification

Skills:
- Demonstrate excellent interpersonal skills to interact effectively and work cooperatively with others
- Ability to communicate effectively and clearly
- Display knowledge and analytical abilities necessary to assess patient condition, evaluate needs, make appropriate referrals and consultation
- Demonstrate proficiency in electronic medical records, e-mail, and internet
- Be able to lift to 30 lbs
