Job Summary
The Destination Care program supports distinguished multispecialty programs at University of Utah Health ( UUHC ) by coordinating efficient and seamless patient care across specialties for comprehensive health care. Programs designated as “Destination Care" have been selected by clinical leaders because they provide unique services, are on the forefront of innovation and technology, and have proven clinical excellence and strong outcomes. The goal of the Destination Care program is to provide optimal patient and provider experiences throughout the healthcare journey.
This Destination Care Patient Coordinator will support the program by identifying and effectively communicating with the patient and the clinical care teams to ensure efficient delivery of care for traveling patients.
Responsibilities
Responsibilities:
- Effectively and efficientlysupport several specialty programs to aid in the coordination of appointments for the programs traveling patient population
- Facilitation of clear care communication with providers and clinical teams
- Monitors and maintains timely and clear communication with patients, via phone and email
- Serve as the main point of contact for patients and provide current travel resource information
- Provides detailed documentation of all communication within EMR
- Ensures proper patient identification and flag assignment for program involvement
- Aid clinical teams with image collection and connection of images to EMR record
- Manages outgoing patient and team documents for accuracy
- Assist with team and program projects as needed

Minimum Qualifications
Bachelor’s degree in a clinical area, health care administration, business administration, a related field, or equivalency (one year of education can be substituted for two years of related work experience).
Two years of progressively more responsible health care experience.
